Electro-Convulsive Therapy (ECT)

For inpatient or outpatient ECT services, preauthorization and peer review must be requested by contacting Carelon. Please refer to the procedures for requesting preauthorization and concurrent reviews for inpatient and alternative services in Section III above. The same procedures, required clinical information and Medical Necessity Criteria requirements apply for ECT. Please call our toll-free provider number with any questions you may have or for assistance in requesting service authorizations for ECT.

Psychological at Neuropsychological Testing

Lahat outpatient Ang mga serbisyo sa pagsusuring sikolohikal at neuropsychological ay nangangailangan ng paunang pahintulot at pagsusuri ng peer. Dapat kumpletuhin at isumite ng mga provider sa Service Manager ang isang Psychological Evaluation Request (PER) form. Ang form na ito ay maaaring i-fax o i-upload sa pamamagitan ng ProviderConnect.

The authorization decision for psychological tests will be provided within two (2) business days of receipt of all clinical information. Authorizations for psychological tests will be in units of 30 minutes. Please contact Carelon for further assistance in completing and submitting these forms. Please note that inpatient Ang psychological testing ay kasama sa inpatient per diem rate, at dahil dito ay hindi nangangailangan ng preauthorization.

Mga Serbisyo ng Methadone

Lahat ng mga kahilingan para sa Mga Serbisyo ng Methadone ay dapat kumpletuhin ang Ulat sa Pagpapanatili ng Methadone. Ang mga serbisyong ito ay nangangailangan ng preauthorization sa pamamagitan ng pagtawag sa Engagement Center o sa pamamagitan ng ProviderConnect gamit ang methadone maintenance form.
