NOTIFYING CARELON OF REPORTABLE EVENTS

Reportable events (formerly called Critical Incidents) are incidents or outcomes involving in-network providers and/or HealthChoices members seeking or receiving services under Carelon administered plans that may require further analysis. They also include events that occur during a patient’s transition to home or an alternative level of care. Tracking of reportable events is a contractual requirement for Carelon with the Counties, DHS, and our providers, so it is important to report an occurrence promptly.

Notification to Carelon that a reportable event has taken place must be documented on a Carelon Reportable Event Form 이메일을 통해 품질 관리 부서로 전송 Criticalincident@carelon.com or by facsimile to 855-287-8491 as soon as reasonably possible, but ideally within one business day of the date you became aware of the event. Written supplemental notes or a copy of clinical records may be requested.

Reportable Events include but are not limited to the following events occurring within any treatment setting:

04 Allegations of sexual or physical abuse/neglect/exploitation by a provider or non-consensual sex between consumers while at a provider site or where services are rendered

06 거주 제공자로부터 2시간 이상 휴가 없이 결석하여 본인 또는 타인에게 위험이 있는 경우 명백히 심각한 위험 없이 2시간 이상 거주 제공자로부터 휴가 없이 결근하고 밀수품, 불법 약물 등을 가지고 돌아오지 않은 경우. 자신이나 다른 사람에게 위험에 처해 있습니다.
07 심각한 성격의 일상적인 환자 치료와 일치하지 않는 바람직하지 않은 사건(의학적 부작용, 만취 등) 중등도의 일상적인 환자 치료와 일치하지 않는 바람직하지 않은 사건
08 기밀 위반  
09 AMA(납치 등)로 인해 위험에 처한 아동과 함께 입원 환자 환경에서 아동 AMA를 받는 부모 또는 보호자 또는 감금 또는 입원의 심각한 위험(자살 생각 또는 불안정한 정신적 또는 신체적 건강 상태를 나타내는 등)에 있는 동안 자발적으로 치료를 떠나는 성인  
10 긴급/긴급 생명 구조 치료 또는 전문 간호(예: 응급실 또는 긴급 치료 방문)가 필요한 시설 또는 제공자 사이트(서비스가 제공되는 곳)에서 발생한 심각한 우발적 부상 시설 또는 제공자 현장(서비스가 제공되는 모든 곳)에서 응급 처치보다 더 많은 치료가 필요한 심각하지 않은 우발적 부상(응급 처치에는 상태 평가, 상처 세척, 국소 약물 적용 및 간단한 붕대 적용이 포함됨)
11 회원에게 심각하거나 잠재적으로 심각한 피해 또는 고통을 초래하는 투약/치료 오류 회원에게 심각하거나 잠재적으로 심각한 피해 또는 고통을 초래하지 않는 투약/치료 오류
12 회원에게 중대하거나 잠재적으로 심각한 피해 또는 고통을 유발하는 약물/치료에 대한 부작용(NMS 등) 중등도 또는 경미한 성격의 약물/치료에 대한 부작용
13 화재, 재산피해, 범행 등 어떤 사유로든 시설에 긴급구조대(소방서, 경찰, EMS 등)가 출동한 경우 Fire setting or property damage at a facility that does not result in summoning emergency services but does require immediate action or repairs to ensure member safety. False alarms are not reportable.
14 Any condition that results in temporary closure of a facility, regardless of whether or not a Carelon member is affected by the closure.  
15 치명적인 무기 소지 및 회원이 시설, 제공자 현장 또는 서비스가 제공되는 모든 장소에 있는 동안 무기를 사용하겠다는 위협  
16 Outbreak of a serious communicable disease, regardless of whether or not a Carelon member is present at the time of the notification.

21 제공자 사이트(또는 서비스가 제공되는 모든 곳)에서 발생하고 잠재적으로 생명을 위협하거나 심각한 의학적 치료(응급실 또는 긴급 치료 방문과 같은 긴급/응급 치료)가 필요한 자해 행위 Self-Injurious Behavior that occurs at a provider site (or wherever services are rendered) requiring medical treatment more than first aid (First aid includes assessing a condition, cleaning a wound, applying topical medications, and applying simple bandages)

Or, SIB that demonstrates a new behavioral pattern of concern
